LICENSE TESTING APPLICATION INSTRUCTIONS
It is important to fill out the application for off-site testing completely. Below are examples of completed applications. If you have any questions, please call the office at (800) 732-8090 and we will be happy to walk through it with you.
Here is an example of a completed application. Use this as a guide when filling out your own application:

Once you have completed the application, mail it along with a passport photo, copy of learner's permit, and a check made out to All-Star Driver for $85, to:
